If you have ever stood in a supplement aisle, desperate to heal your gut, you already know this feeling. The overwhelm and marketing claims take over, and you leave the store more confused than ever with a bag full of supplements that may or may not work. When it comes to IBS, digestive enzymes are almost always on the list of targeted supplements to heal your gut. But the question is: are they needed, and when are they appropriate to take?

As a Registered Dietitian specializing in IBS and SIBO, the topic of digestive enzymes comes up often. Some clients have tried them and felt no improvement. Others have never heard of matching an enzyme to a specific food trigger, and grabbed whatever was on the shelf. In this post I will discuss when I recommend enzymes for IBS and when I don’t, how to choose the best enzyme, and my favorite brand of enzyme.
What Is a Digestive Enzyme
Digestive enzymes are proteins your body produces to break food down into pieces small enough to absorb. Your pancreas, stomach, and small intestine are all involved in making different enzymes, each built for a specific job.
The body’s digestive system is more complex than most people may realize. There is no single, all-purpose enzyme that breaks everything down. Amylase is built for starch. Lipase is built for fat. Protease is built for protein. Lactase is built for lactose, the sugar in dairy. Alpha-galactosidase is built for the galacto-oligosaccharides found in beans, lentils, soy, and certain vegetables. Each enzyme recognizes one specific molecule and leaves everything else alone.
When you do not make enough of a specific enzyme, the food that the enzyme was responsible for breaking down travels through the gut undigested rather than absorbed.
When I Recommend Digestive Enzymes with Food Intolerances
Many people with IBS are also managing food intolerances, and more specifically, FODMAP sensitivities. FODMAPs are fermentable carbohydrates that the small intestine absorbs poorly, even in people without IBS. For someone without IBS, this reaction goes unnoticed. The difference, though, is that when someone has IBS, visceral hypersensitivity kicks in and amplifies the fermentation, leading to undesired symptoms such as gas, bloating, pain, and irregular bowel movements.
The good news is that there are studies that support specific digestive enzymes can break down certain FODMAPs. A randomized, placebo-controlled crossover study found that alpha-galactosidase reduced gastrointestinal symptoms in IBS patients after a meal containing galacto-oligosaccharides(Tuck et al., American Journal of Gastroenterology, 2018). A separate trial confirmed it reduced the symptom spike that typically follows GOS-containing foods like beans and lentils (Di Stefano et al., Digestive Diseases and Sciences, 2007). For fructose malabsorption, a double-blind study found that xylose isomerase reduced nausea and abdominal pain when taken alongside fructose consumption (Komericki et al., Alimentary Pharmacology & Therapeutics, 2012). Lactase has decades of research behind it. For lactose specifically, it is still one of the most common triggers Misselwitz et al., Gut, 2019).
None of this evidence means much, though, without knowing your own triggers first. As discussed earlier, enzymes are specific to the food that they target. A structured elimination diet under the supervision of a Registered Dietitian is one of the most reliable ways to identify those triggers. From there, you can determine which enzyme will actually benefit you.
