Before you automatically increase your iron dose, it’s worth asking a bigger question:

WHY aren’t your iron stores improving?

Low ferritin isn’t always as simple as “you need more iron.” Absorption, stomach acid, nutrient cofactors, B12 status, blood loss, inflammation, and iron utilization can all be part of the picture.

If I were dealing with persistently low ferritin, I’d want to look at the root cause, not just chase the number with more supplements.

You’ve done the SIBO protocol. Symptoms came back You’ve done the SIBO protocol. Symptoms came back anyway. Here’s a connection almost nobody makes: H. pylori. 🦠

H. pylori survives your stomach acid using an enzyme called urease, which converts urea into ammonia. That ammonia neutralizes the acid right around the bacteria — creating a buffered pocket it can live in. In a chronic infection, that constant neutralizing can lower how much acid your stomach makes overall.

Here’s why:
→ Stomach acid is your first line of defense — it kills off bacteria that come in with food before they reach your small intestine. Less acid means more bacteria surviving the trip downstream.
→ Low acid also means protein doesn’t break down properly, and the signals that keep your gut moving don’t fire the way they should.
→ Slower motility + more surviving bacteria = exactly the environment SIBO needs to take hold, or keep relapsing.

Not everyone with H. pylori develops SIBO, and not every SIBO case traces back to it. But if an active infection is still suppressing your acid, treating the overgrowth alone won’t fix what’s letting it come back.

If your SIBO treatment has worked short-term more than once and then fallen apart, this is worth ruling out.

IBS and food intolerances are not always the same IBS and food intolerances are not always the same thing, even though the symptoms often overlap.

If you have IBS, there is a real chance a food intolerance is layered underneath it, most often tied to FODMAPs. The fix is not eliminating everything. It is identifying your unique trigger. Once you know what they are, digestive enzymes can be a helpful tool, but only when matched to that specific trigger:

🥛 Lactose → Lactase
🍎 Fructose → Xylose isomerase
🫘 GOS (beans, lentils, certain vegetables) → Alpha-galactosidase
🧀 Histamine → DAO enzyme

This is why I point clients toward @intoleran_usa when enzyme support makes sense. Their formulas are high-dosed and designed for specific food intolerances, not a one-size-fits-all blend.
